Contractor Agreement
Review and sign your Forge Logistics LLC contractor agreement.
By submitting this application, I ("Contractor") agree to the following terms with Forge Logistics LLC:
Independent Contractor Status. I understand and agree that I am an independent contractor, not an employee of Forge Logistics LLC. I am responsible for my own taxes, insurance, and equipment unless otherwise specified in a written agreement.
Payment Terms. I understand that I am paid based on hours worked as approved by the client. Payment will be issued within thirty (30) days of the date the client approves my hours ("Net 30"). Forge Logistics LLC is not obligated to pay for hours that have not been approved by the client.
Late Payment. If payment is not received within ten (10) business days after the 30-day period described above, I may contact Forge Logistics LLC's administration to inquire about the status of payment.
Work Standards. I agree to maintain professional conduct, arrive on time, follow all site safety protocols, and perform all assigned duties to industry standards. Failure to meet these standards may result in removal from Forge Logistics LLC's contractor roster.
Confidentiality. I agree to keep all client, venue, and event information confidential unless required by law to disclose.
Cancellation. I understand that cancellations within 48 hours of a scheduled call may result in removal from future bookings at Forge Logistics LLC's discretion.
Accuracy of Information. I certify that all information provided in this application is accurate and complete. Falsification of any information may result in immediate termination from the Forge Logistics LLC roster.
Background & Verification. I authorize Forge Logistics LLC to verify the information submitted, including employment eligibility and provided documentation.
